Health Support and Voluntary Groups invited to have your say on Sutton Health and Care Programme
For example, on one of the wards in St Helier Hospital where patients have required the input of multiple services, there are now twice-weekly meetings of acute health professionals, physiotherapists, occupational therapists, community health providers, social care staff and continuing healthcare assessors to enable fast patient discharge. This is just one of the many changes taking place across Sutton to enable better outcomes for our people.
The Sutton Health and Care Programme has involved and continues to involve patients and the public in helping design how the new service will deliver better patient outcomes in Sutton. The programme is now looking to involve more representatives from health support and voluntary groups based in the borough.
